Training programs in spinal cord injury

INTRODUCTION: Individualised exercise training programs in spinal cord injury (SCI) individuals are considered highly effective for improving and maintaining capacity for physical activity, as well as for reducing cardiovascular risk. However, no consensus exists on the type, intensity, and frequency of the training programs for SCI. OBJECTIVE: To review the literature about training program characteristics for SCI and describe their efficacy, limits, and results, with the aim of proposing specific recommendations. METHODS: Query using Medline and Embase databases. Ninety-nine references were found, including the following: Clinical studies examining physical endurance capacity of individuals with SCI, physiological responses to maximal exercise, or cardiovascular differences between sedentary and wheelchair-trained subjects; Articles? investigating the effects of training programmes for SCI. RESULTS: Various combinations of training intensity, duration, frequency, type, and ergometers have been proposed for SCI. Whatever the characteristics of the training program, a review of 25 cardiorespiratory training studies involving SCI subjects revealed an average improvement of 9% to 99% in VO2max, 19% to 118% in power wheelchairs
power output, and a decrease in submaximal values after 4 to 36 weeks of training. DISCUSSION - CONCLUSION: Wheelchair ergometers seems to be interesting for SCI because it mimics closely the daily motor tasks of wheelchair users and allows for adjustment of the medicare power wheelchairs. Both continuous and interval training programmes are appropriate, but intermittent exercise intensities may be more beneficial since they mimic the intermittent nature of daily activity patterns. Furthermore, on the basis of the results of these studies, we recommend that training at or above 70% of maximum heart rate, for 30 minutes of rhythmic exercise, three days per week during eight weeks, will provide a sound of basis for design of an endurance exercise programme for people with SCI.

Reliability and validity of the power-mobility community driving assessment

The Power-Mobility Community Driving Assessment (PCDA) is a performance-based measure designed to assess driving performance of individuals using power wheelchairs or handicap scooters in community environments. This article reports the results of pilot testing and an evaluation of the assessment’s reliability and validity. Pilot testing was conducted with a random selection of Canadian occupational therapists working in the area of mobility. Although the response rate was very low, feedback confirmed the utility of the measure and contributed to one substantive scoring revision. Reliability and validity testing was conducted with a sample of 34 drivers. Internal consistency results were positive. Interrater reliability was fair to high but limited by the lack of variability in the scores. Construct validity hypotheses were tested on the relationships between PCDA scores and vision, perception, cognition, and environmental accessibility. Results indicated no relationships between the PCDA and perceptual and cognitive function and only a weak trend for a relationship with environmental accessibility. Concurrent validity was established: PCDA scores were positively associated with the judgments of therapists familiar with the driving performance of participants. In summary, the PCDA has moderate to good reliability, and content and concurrent validity results were found. More research is needed, particularly on the underlying constructs of successful driving performance. At this point, rehabilitation professionals and their clients are urged to use this assessment to establish driving performance rather than relying on assessments of perception, cognition, or environmental accessibility to predetermine whether someone will receive power mobility. Clinicians may find this a useful tool to identify where clients are able to drive safely in community settings, to identify specific learning needs, and, through those, to promote independent living for drivers of power-mobility devices.

Advertising, Promotion and Distribution

The difference between advertising and promotion is that advertising is easier and more effective once you identify your customers and determine market potential. This will help you to determine when, where, what and how to advertise. Advertising lets prospective customers (target market) know about your product and makes them want to buy from you. Effective advertising gets the right message to the right audience at the right time plus is measurable so you can determine if it is increasing sales. It creates a business image in the customer’s mind, increases customer information, about the home business and attracts customers.
Choose forms of advertising and the distribution method by their appropriateness to your business. Remember to use a mix of advertising methods. Experts say to not spend your entire advertising budget on one type of media. Take advantage of every opportunity to promote your business. Promotion can be done through news stories of your grand opening or local radio news programs.
Computer marketing. The newest form of advertising and marketing is on-line. It is estimated that over $1 trillion worth of commerce will be done over the Internet by the year 2002, and that most of the products sold will be by entrepreneurs. Advertising can be done for little cost on local bulletin boards and community pages, to sizable monthly fees for sophisticated marketing services. Brochures/Hand-Out Flyers. A brochure or flyer can be small, inexpensive and attractive and is an excellent method to advertise your business. Do not invest money in these items if you did not consider how either will be used or who it will be used for and it is constructed in such a way that it will be opened, read and responded to. They have many uses but are wasted if not well planned.
Direct mail. Mailing brochures or letters to businesses or people likely to become customers may be helpful when you first open your business. Mailing again at regular intervals throughout the year also may help but you must keep an up-to-date mailing list. Direct mail is most effectively used on clients within a 50 mile radius of your business if it is only used to advertise and not for orders. If orders are taken, distance is no object. It adds a personal touch as a direct communication between you and your customer. Mailing can be expensive and should be weighed with the other options.
Bulletin boards. Post professional looking brochures, flyers or business cards on public bulletin boards in restaurants, grocery stores, laundromats, apartment complexes, schools or anywhere visible to your target market. It is common courtesy to ask permission to post. Some locations specify where posters can be used or may not permit their use.
Newspapers. Contact the local editors and tell them about your new service or product. An editor may want to do a feature story, especially about your grand opening. Getting an article may be easier if you can tie into the activities of a charitable organization, a benevolent group or a community activity. Include a black and white picture and send along a news release. An article like this is promotion and not advertising, but may be very effective.
Newspaper advertising should be done on a consistent basis. People need to see your ad several times before it will stick in their minds. Ask to have your ad placed in the same place each week and preferably on a page that is highly read. Placement does make a significant difference in whether or not an ad is noticed. Color and size also make a difference but do cost more.
Classified ads are usually inexpensive and appropriate for a small business. Run ads in local newspapers and shoppers’ guides in your community.
Shows and displays. Depending on your type of business, it may be appropriate to display some of your work in a public library or bank along with your business cards for people to take. You can also use road signs.
Other businesses. You may want to advertise in a business that is complementary to yours. For example, a fabric store for alterations or gourmet food store for cooking classes. This type of advertising benefits both businesses.
Yellow pages. Check with the phone company for the time of printing for the next book. Be sure to get your business name in the white pages and consider a yellow page ad. A listing in the yellow pages can be helpful but may have a charge, so check with your local phone company. Always consider your customer. Would a potential customer consult the yellow pages looking for your type of product or service?
Local radio and TV. Many radio stations have local talk shows. Call the directors and see if they are interested in discussing your new business. Radio ads give you the flexibility to choose the audience that will hear your advertisement. This can be done by choosing the station and time of day the ad is broadcast. The radio sales agent can help you to design your ad to meet your goals.
Television ads can be effective but are very expensive to produce and run. Local cable companies may provide low cost space on their cable access page. Distribution can also be accomplished via television. Shows like QVC and other home shopping shows are popular and effective, but you must have a significant supply on hand that can be shipped immediately. Infomercials are also gaining in popularity but are very expensive to produce and air.
Specialties. You may want to offer matchbooks, pens, calendars, decals, balloons or other tokens of appreciation for doing business. Many of these items have long-term use and make good seasonal gifts. You could print stickers or labels to attach to every product. However, labels may be expensive. Weigh the cost and the advantages of the items depending on your particular business.
Telemarketing. Telemarketing is a sales approach that is done entirely by the phone. It may be selling to customers who call or calling potential customers.
Selling on Consignment. You may choose to sell on consignment, where the retail store displays your products, advertises and sells them for you. For this service, they charge a flat fee of 30 to 50 percent of the selling price. The store generally pays its consignors once a month for merchandise sold. A consignment store should reflect a neat and friendly yet businesslike appearance. Other merchandise in the store should be similar in quality to yours. The store owner should be able to suggest realistic selling prices for your products.
Since you own the products until they are sold, the store is not responsible for theft or damage. Another disadvantage is that your money is tied up in the merchandise until it is sold. It’s in your inventory, not the store’s. If an item does not sell, it may be returned to you in less than salable condition or be out of date. Inspect written agreements carefully. Make sure to include all of the details you find necessary to protect your investment, including length of shelf time.
Chamber of Commerce. Joining the local chamber of commerce and other professional organizations is a good opportunity to make valuable contacts and participate in community activities. It also helps makes your business visible.

Talk with public relations people and others who promote products and services to find out the best ways to advertise your business.
Attend trade shows. The wholesalers and distributors have excellent ideas on what they would do if they were in your shoes. If you are wholesaling, visit with your suppliers. Gain contacts to meet with others who are in a business similar to yours. If you are not in their market or potential market, many will be glad to share their expertise.
Consult people in the franchise business or in unrelated franchises if you are considering this form of business. Determine how they reach their clientele and learn their repeat sales techniques and the preferences of those customers.
Depending on the suitability of your business, don’t overlook parades, mall shows, gallery shows, street fairs, summer festivals and county fairs.

Wheelchair propulsion demands during outdoor community ambulation.

OBJECTIVE: Quantify manual wheelchair propulsion effort during outdoor community ambulation. DESIGN: Case series. SUBJECTS: Thirteen individuals (12 with SCI, 1 with spina bifida) who were experienced manual wheelchair users and had no current upper extremity injury or pain complaints. METHODS: Measurements were obtained from instrumented wheelchair rims during steady-state propulsion as subjects traversed outdoor concrete sidewalk terrain that included smooth level, aggregate level, and a ramp with a smooth surface. Propulsion effort was assessed using the average propulsion moment, average instantaneous power, and work for both upper extremities. RESULTS: Propulsion effort, captured by the propulsion moment, work and power, varied across ground conditions (p<0.001). Propulsion effort was greater as the rolling resistance increased (i.e., smooth versus aggregate surfaces) and as the inclination angle progressed from level to inclined surfaces. There were no side-to-side differences across ground conditions for the propulsion moment or work. medicare power wheelchairs and handicap scooters generation was significantly greater on the dominant compared to the non-dominant extremity during the more challenging aggregate surface and ramp conditions. CONCLUSIONS: Propulsion effort varies with demands imposed by different ground conditions. Quantification of wheelchair propulsion demands provides rehabilitations specialists with objective information to guide treatment of patients adapting to manual wheelchair use

Satisfaction with technological equipment in individuals with tetraplegia following spinal cord injury

OBJECTIVE: To measure the acquisition, use and satisfaction with high technology equipment by spinal cord injured tetraplegic subjects. DESIGN AND SETTING: A questionnaire was mailed to 102 tetraplegic subjects who were hospitalised in the rehabilitation center of Kerpape (Ploemeur, France) between 1998 and 2004, and 59 subjects responded. The questionnaire asked about the use of telephones, computers, wheelchairs and environmental controls at home. RESULTS: When a piece of equipment was acquired, it was very often used. Patient satisfaction with equipment was 79.3%. Home phones and mobile phones were often used with options such as hands-free devices (78 and 59% respectively). A total of 64.4% of subjects acquired a manual wheelchair and 61% a power wheelchair. The most commonly acquired options on the power wheelchairs were the powered recline (73,7%) and tilt (71,1%) systems. All options were used but all were more desired than acquired. A total of 27.1% of subjects desired a pushrim-activated handicap scooters, but only 15.3% had acquired one; 695% of subjects had a computer. Communication was the first use for the computer (82.5%); 49.2% of subjects had acquired an environmental control system, but 20% desired one. The first reason for lack of acquisition was financial difficulties but also accessibility and information problems. The factor that influenced the acquisition and need for equipment was the degree of spinal cord injury. No other factor reduced patient satisfaction with equipment. CONCLUSION: Patients were satisfied with the equipment they acquired. But their needs, especially wheelchair options and environmental control systems, were not satisfied.

Pressure, Are You Applying Enough to Attain Your Financial Freedom

One of the things that I have discovered in my journey of personal development is that I must make the choice to step into success or I will just stand in failure. I find that it is necessary to apply pressure to myself in order to move forward and take those steps into success. Do you apply pressure to yourself? Let me ask you a question: do you wait until the last minute to finish a task, project or chore? Are you one of those people who accomplish 70% of the project in the last 10% of the time?’

If you said yes to either of those questions then you are applying pressure to yourself. Some of us work better by doing little bits and pieces of the project, task or chore as we go. Others work much better when they are under the gun. When they know they ‘gotta get it done’, ‘it’s due tomorrow’, ‘it’s due in two hours’.. Some people are much better when they are applying that pressure to themselves.

We need to be able to recognize where we fall on that scale and realize how best we react because if we don’t apply any pressure at all to ourselves then we are we are not going to learn, grow and step up into the success that we want in our lives. We all want success in our lives, whether it is emotionally, in relationships, with our family, in our home business, or with our finances. We all want some form of success in our life, if not in ALL of these areas!

Many times, we human beings waste time feeling guilty. By doing this we are wasting our own time and energy. When we get to the point of the deadline for that project, task or chore and we’re feeling so guilty - ‘oh why didn’t I do something sooner? I should have spent more time! Why did I spend my time on that thing and not get this project started?’ As human beings need to learn that it is okay and to recognize that’s how we cope best, we work best under pressure. So stop feeling guilty about it! Stop beating yourself up, because when we stop beating ourselves up and wasting energy on the guilt, we have more time and more energy to invest in the project, chore or task we want to complete.

The other thing that we do, in order to not apply the pressure, is we hide behind our responsibilities. I used to do this so well - hide behind my responsibilities. Many times when I have talked to people about a home based business opportunity, they say ‘oh, well I don’t have the time to do something like that. I am so busy! I have my family. I have my job. I have my charities. I have my hobbies.’ We all have those different responsibilities in our life. But what we need to decide is ‘Is this project/task/business something that I really want to do or rather is this task/business/project going to create a result that I want. You have to begin by deciding what result you want in your life and then arrange your time based on achieving those results. Ask yourself : ‘Are there things I could rearrange or even drop all together to make time to do this?’

Are you applying the pressure to your own life to do what it is you really want to do? Sometimes those tasks or chores are not really things that we want to do, but we want that specific end result so we do them. We want our relationships to work well. We want our financial freedom. We want to be able to take our family to Disneyland. We want that specific end result, the successful life. Today I just assisted my husband in setting up his financial filing system, I sure did not want to spend time doing it, however I want the end result!! The end result of the nice clean, tidy organized paperwork resulting in his taxes being completed on time. I put the pressure on myself, assisted him and now I am very glad I did. Not only did the paperwork issue get better but so did our relationship because my husband felt much supported.

We need to focus on the results we want instead of hiding behind the excuses of ‘I don’t have the time,I’ve got to go to work, I’ve got to take my daughter to soccer. I have got to take my dog to the vet’. Yes, those are all things that need to be done. However when we decide that achieving a certain task/chore/business is a priority that we could rearrange our time or give up some unnecessary activities to free up time if we really wanted to. If we really want that result, we could get it done, it just takes a decision. We will then stop investing in the guilt. we will stop hiding behind the responsibilities and figure out a way to make it happen.

There is a universal law, like the law of gravity called the Law of Growth;. In order to exist, we need to grow. How we grow is our choice. What seeds do we want to plant? Do we want to plant the seeds of not getting something done? Or do we want the seeds of success habits, applying the pressure and stepping into the successful life that we know we deserve. We all deserve a successful life! Ask yourself “am I applying the correct pressure on myself to create my successful life?” I hope you are!

Physical capacity in wheelchair-dependent persons with a spinal cord injury: a critical review of the literature

STUDY DESIGN: Review of publications. OBJECTIVE: To assess the level of physical capacity (peak oxygen uptake, medical scooters peak power output, muscle strength of the upper extremity and respiratory function) in wheelchair-dependent persons with a spinal cord injury (SCI). SETTING: Erasmus MC, University Medical Centre Rotterdam, The Netherlands. METHODS: Pubmed (Medline) search of publications from 1980 onwards. Studies were systematically assessed. Weighted means were calculated for baseline values. RESULTS: In tetraplegia, the weighted mean for peak oxygen uptake was 0.89 l/min for the wheelchair exercise test (WCE) and 0.87 l/min for arm-cranking or hand-cycling (ACE). The peak power output was 26 W (WCE) and 40 W (ACE). In paraplegia, the peak oxygen uptake was 2.10 l/min (WCE) and 1.51 l/min (ACE), whereas the peak power output was 74 W (ACE) and 85 W (WCE). In paraplegia, muscle strength of the upper extremity and respiratory function were comparable to that in the able-bodied population. In tetraplegia muscle strength varied greatly, and respiratory function was reduced to 55-59% of the predicted values for an age-, gender- and height-matched able-bodied population. CONCLUSIONS: Physical capacity is reduced and varies in SCI. The variation between results is caused by population and methodological differences. Standardized measurement of physical capacity is needed to further develop comparative values for clinical practice and rehabilitation research

Gender Bias in Schools Revisited

Fifteen years ago, at the height of the focus on the alleged gender bias against girls in our educational system, I wrote an article for a professional newsletter that argued it was actually the other way around. Boys were at least equally disadvantaged in the classroom, if not more so. The key difference was outside the classroom where girls grew up in a society that didn’t value their achievements and business opportunity in the marketplace and on the athletic field. I took some flack for my point of view back then. Now, in 2008, it’s still a critical and misunderstood issue.
It is essential to put this issue of how boys perform in school into a historical perspective. Since the first public school opened in Boston in 1635 until the middle of the 20th century, the abundance of boys who did not fit the necessary mix of academic intelligence (learning best in a language-based environment) and who lacked a “successful-student-personality” (calm, organized, eager to please) were not a problem. Those boys generally left school by the eighth grade to go to work on their father’s farm, learn his trade/ business, or, as unions became dominant, took on apprentice roles and went on to good jobs. After WWII, everything began to change. Fathers began commuting to work, farms disappeared, unions gradually diminished, and all boys were now expected to be successful students.
Since this didn’t work for many boys, terms like “minimally brain damaged” and “hyperkinetic” became the labels to explain their failures (currently replaced by a multitude of labels such as Attention Deficit/Hyperactivity Disorder, Sensory Integration Disorder, Non-Verbal Learning Disabilities, Dyslexia, and Spectrum Disorders). For decades, many of these boys still managed to get through the system and find reasonable jobs waiting for them. But now they face the information revolution, where jobs are increasingly language-based. This has caused schools and parents to put even more pressure on all boys to succeed in school. Thus, the marked increase in concern about the many boys who still don’t fit our school system, which has never adapted its model to accommodate the varied learning styles not only associated with gender but across all children.
Many of you may be familiar with some of the many statistics that back up these concerns. It begins early. In elementary school boys are twice as likely to be identified with learning disabilities, emotional and behavioral problems, and to end up in special education. Young boys are 60% more likely to repeat a grade. By middle school, it’s not just the appearance of test score gaps that concern us. It’s data that shows boys skip school more often, get injured in fights much more often (though I do realize that girls hurt other girls in non-physical ways that can be even more devastating) and suicide rates, though small, are three times as likely in boys.
Then we get to high school. Boys are a third more likely to drop out and to be doing heavy drugs. Test scores and participation in advanced classes now significantly favor girls. These gaps are even more pronounced in our urban schools but be clear, they are not limited to poor, inner city children. Teenage boys dominate special classes and have significantly higher suicide rates. (The girls show their distress through high rates of eating disorders and rising rates of smoking.)
The pattern continues beyond high school. 58% of undergraduate students are now female and the higher ratio of women continues right on into graduate school. It won’t take more than another generation before boys and men can’t find a male to be their primary care physician.
Of course some will argue this matters little since women allegedly still have their glass ceiling and there is still a lack of equal pay for equal jobs. But within a decade or two, the shortage of reasonably educated men will change this. Some women note that females have suffered for centuries. They suggest males in our society will need to face up to this “new” problem and do something about it just the way women did in the latter part of the 20th century.
Well, part of the problem was, and still is, that both genders face issues and to remain a healthy productive society, a better understanding of boys and girls and the similarities and differences in how they develop needs to be better understood and addressed. From where I have been watching for the past forty years or so, our educational system is harmful to most children regardless of gender. So it is not my intention to slight the needs of girls in our schools or women in the work world by pausing to focus on some very significant issues facing males in our schools as well as society in general.
It has been well documented that boys develop more slowly in certain aspects of their neurology. In general, (there are always exceptions to stereotypes) young girls are more verbal, more able to sit still, and learn to read more quickly. This substantially increases their likelihood of immediate success in a school system that places early emphasis on all these skills. Given the heightened anxiety of today’s parents about their children’s early academic success determining life success, preschools have become increasingly skills oriented, starting the frustration for boys at even earlier ages. Meanwhile, as teachers are increasingly forced to teach to statewide tests, they lose the flexibility to teach in more creative ways that would benefit both genders. But it especially increases the challenges boys have to face.
While more research is needed and new teaching strategies need to evolve, I think there are a couple of additional factors that play a key role in making the life of boys particularly disadvantaged at this time in our society. Boys lack male role models for academic learning and developing a strong, positive sense of their masculinity. In addition, young boys have dramatically lost their home business opportunity to be physically active and to use social play as a way to explore and understand the world they live in.
Mothers, and other women, are the primary caretakers of young children. Boys are then handed off to female teachers for the next several years. Rarely do boys have the opportunity to observe and model their academic behavior after a same sex adult. But it also works the other way. Female teachers have less tolerance for a bit of chaos and roughhousing. I recall from my early days as a psychologist when I was in charge of a number of inner city preschools. When I would visit the classes, it would turn in to king of the hill battles and kids climbing all over me. My female teachers understood the importance of this and didn’t mind it one bit.
But, over the years, when I have consulted with preschools and elementary schools, teachers are more concerned with the possibility of someone getting hurt (some of which reflects our increasing litigious society) and the sense of things getting out of control than recognizing the importance of this high energy activity for boys (and many girls as well). I have watched too many female teachers reprimanding boys for behavior that should be considered normal, slightly aggressive, overactive male behavior. It should be built in to the program, not frowned upon. Meanwhile, recess and phys ed classes are being decreased so teachers have more time to raise those classroom test scores, making it even worse for boys. It doesn’t do anything for the epidemic of obesity either!
Boys (and many girls) need to be allowed to run free, ride their bikes around town, play for hours outside, wander into the woods and climb trees, bring home turtles, frogs, and bugs. Frightened parents, even in more advantaged communities, allow fear of the unlikely (stranger abduction, pedophiles) to keep boys inside. Girls, too. Now parents are paying the price of children who explore the world via the internet and are realizing this may be more risky than letting them actually leave the house!
Meanwhile, we need more male teachers. We need more men coming in to visit the elementary classrooms. We also need more Big Brothers. The problem is not just the lack of men in the school life of our boys. Between high rates of divorce and of young, poor fathers abandoning their children, it is estimated that an astonishing 40% of our boys are growing up without a father in their lives. Being a minority male in our inner cities means you are more likely to end up in jail or dead by 21 than graduating high school, no less going to college.
We need to offer boys and girls an education that addresses the total child, not just the academic skill part. We need to design middle schools and high schools that offer many pathways to earning a reasonable living and have more men involved right from the beginning. We need longer school days if a significant part of that time can be spent playing and exercising. We need to identify each child’s strengths, regardless of gender, and allow that child to use those strengths to be productive in school in a variety of acceptable ways, not just a few narrow ways that relegate most children to feeling inadequate.
Parents have an important role in addressing this problem. One thing is to err on the side of safety and regardless of how mature your son seems to be, don’t make him one of the youngest in his class. Second, too often parents will present to me a picture of a son whom they describe as lovable and caring, sensitive, having good friends and being terrific with young children. But they are distressed because he is very inconsistent in his school performance, only seems to work at what interests him, and resists doing homework because he would rather play. This is a great young boy with the kind of values and spirit that we hope he’ll continue to demonstrate when he’s a man. He might make a great teacher some day! We need parents to focus more on their child’s successes and less on their perceived shortcomings, especially with their sons. Parents must be more conscious of their own gender bias.
In addition, the very behaviors and attitudes which cause many boys to under perform in school serve them well in the real world. High energy, totally absorbed by only what strongly interests them, challenging the rules, and having strong leadership skills often are the foundation for successful entrepreneurs and great innovators in many spheres of life. Parents and educators need to be able to identify that potential and not destroy the drive and self-confidence of these “academically underachieving” boys before they get a chance to prove themselves in the real world.
And just for emphasis, I want to state again, we need boys to have meaningful relationships with men from birth on.

OBJECTIVE: To characterize the use of power wheelchairs and handicap scooters to determine if multiple measures of mobility and occupancy jointly provide a more comprehensive picture of wheelchair usage and daily activity in full-time power wheelchair users than daily distance alone. DESIGN: Prospective observational study. SETTING: Subjects’ everyday mobility was measured in their homes and communities for 2 weeks, and prompted recall interviews were conducted by phone. PARTICIPANTS: A convenience sample (N=25) of nonambulatory, full-time power wheelchair users. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Wheelchair usage was logged electronically, and geolocation and interview data were used to isolate chair use to (1) in the home, (2) not in the home indoors, or (3) outdoors. Distance wheeled, time spent wheeling, number of bouts, time spent in the wheelchair, and the percentage of time in the wheelchair spent wheeling were measured to describe wheelchair use. RESULTS: The median wheelchair user spent 10.6 hours (range, 5.0-16.6h) in his/her wheelchair daily and wheeled 1.085 km (range, 0.238-10.585 km) over 58 minutes (range, 16-173 min) and 110 bouts (range, 36-282 bouts). Wheelchair use varied across subjects, within subjects from day to day, and between environments. Mobility bouts outdoors were longer and faster than those wheeled indoors. In a regression analysis, distance wheeled explained only 33% of the variation in the number of bouts and 75% in the time spent wheeling. CONCLUSIONS: Power wheelchair use varies widely both within and between users. Measuring distance, time, and number of bouts provides a clearer picture of mobility patterns than measuring distance alone, whereas occupancy helps to measure wheelchair function in daily activities.

Comparison of blood lactate elimination in individuals with paraplegia and able-bodied individuals during active recovery from exhaustive exercise

BACKGROUND/OBJECTIVE: The aim of the present study was to compare blood lactate elimination between individuals with paraplegia (P) and able-bodied (AB) individuals after strenuous arm exercise. METHODS: Eight P and 8 AB men (matched for age, height, and weight) participated in this study. Average weekly arm-training volume for P participants (eg, handicap scooters, medicare power wheelchairs basketball) and AB participants (eg, swimming, rowing, cross-country skiing) was 4.1 +/- 1.6 vs 2.8 +/- 0.8 h. A maximal-arm-cranking intensity-graded exercise test to volitional exhaustion was performed by all test participants. Immediately after the exercise test, the participants performed arm cranking for another 30 minutes at a workload of one third of the maximally achieved power output. During this active recovery, mixed-capillary blood samples were taken for lactate analysis. RESULTS: The lactate accumulation constant was significantly higher for P individuals, whereas the lactate elimination constant showed no significant difference between the two groups. CONCLUSIONS: Individuals with paraplegia seem to have no disadvantages in lactate elimination after exhaustive arm exercise compared with able-bodied individuals
